Medical school admissions have undergone significant changes over the past decade. Acceptance rates have become considerably more competitive, making it increasingly difficult for applicants to gain admission.

In 2022, medical schools received 1,099,071 applications and accepted/enrolled 22,671 students (acceptance rate: 3.52%). For the academic year 2025-2026, applications reached 1,040,848, and enrollments were 23,062 (acceptance rate: 3.81%).

Medical school admission patterns vary significantly by gender and residency status. In 2022, male applicants totaled 493,334 with 604,799 female applicants. In-state applicants were 249,412 compared to 849,659 out-of-state applicants.

By 2026, male applicants decreased to 454,187, while female applicants decreased to 580,141. In-state applicants totaled 230,438 and out-of-state applicants reached 810,410.

In 2022, the average acceptance rate for medical schools was around 3.52%, with some highly competitive institutions accepting less than 10% of applicants. By 2026, the average acceptance rate had increased to approximately 3.81%, reflecting decreased competition among applicants.

This rise in acceptance rates has made it less challenging for prospective medical school students to gain admission to their desired schools, particularly at top-tier institutions.

For the academic year 2025-2026, on average, public medical schools receive approximately 5,415 applications per institution over 97 ABA-accredited public medical schools, while private medical schools receive 8,057 applications per institution over 64 private medical schools.

In 2022, public medical schools received 556,226 applications with an average acceptance rate of 4.46%, while private medical schools received 542,845 applications with an acceptance rate of 2.02%. By 2026, public medical school applications decreased to 525,230 with an acceptance rate of 4.81%, while private medical school applications decreased to 515,618 with an acceptance rate of 2.32%.

Medical school admissions show distinct patterns by gender. In 2022, male applicants totaled 493,334 with 10,097 matriculants (acceptance rate: 2.05%), while female applicants numbered 604,799 with 12,555 matriculants (acceptance rate: 2.08%)

By 2026, male applicants decreased to 454,187 with 10,255 matriculants (2.26% acceptance rate), while female applicants decreased to 580,141 with 12,675 matriculants (2.18% acceptance rate).

By 2026, in-state applicants decreased to 230,438 with 13,752 matriculants (5.97% acceptance rate), while out-of-state applicants decreased to 810,410 with 9,310 matriculants (1.15% acceptance rate).
